Hospital to home care services support individuals returning home after hospitalisation. We work with the hospital and the individual's family to ensure a gentle, well-organised return home; providing comfort and reassurance during any recovery.
Services are provided for elderly individuals as well as any adult requiring some support when returning home from hospital.
Coming home from hospital has its challenges for the individual needing support as well as family members and whichever health and other professional services are involved in care package needed at home.

We are very pleased with the care that is being given to our mother. It has made her life much more comfortable, and that has helped her feel more relaxed. The addition of transport to, and support with, appointments away from home has also been a great help as well. It is just a shame that we cannot set up a direct debit to pay for the services and we’ve encountered a small issue contacting the office to make payment by phone.

Our hospital to home service is to get you back home in a timely fashion, safe and well looked after and to settle you back in once you're at home, ensuring your safety and making sure any new care needs are not left unattended.
This service is not to take you to hospital appointments and back. Our companionship care services are able to support with trips to the dentist, doctor or hospital appointments. Please have a chat with us about your needs and we'll see what we can do to help.
