“This Is Me” is a leaflet designed for individuals receiving professional care who are living with dementia, experiencing delirium, or facing other communication challenges.
A Tool to Support Person-Centred Care:
“This Is Me” helps carers understand the individual beyond their care needs. It allows important personal information to be recorded when someone may find it difficult to communicate details about themselves.
Developed by the Alzheimer's Society, this simple but powerful booklet helps individuals share their interests, preferences, and life story, allowing carers to provide more personalised, person-centred care that respects each person’s identity and dignity.
